Orthopedics

For patients recovering from orthopedic surgery, we provide home health care designed to improve mobility and function and return you to a more active life.

Our philosophy is that healing can begin even before orthopedic surgery. That’s why we educate you about what to expect during your recovery. We’ll assist your surgeon in developing a progressive rehabilitation plan then start your physical therapy as soon as possible after you leave the hospital. Our orthopedic clinician will continually assess your ability to walk and perform other activities safely. This allows adjustments to your care plan to be made quickly and effectively, and ensures you’ll meet your treatment goals.

Cardiopulmonary program

Exceptional clinical training and the use of technology to standardize our approach distinguish our Cardiopulmonary Programs.

We care for patients with Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), hypertension (high blood pressure) and other heart and vascular diseases. Our care plans, developed with your physician, include comprehensive assessment, patient education and self-management – the knowledge, tools and motivation you need to comply with your doctor’s orders. As a result, you’re more likely to achieve your health care goals, and less likely to need urgent or emergency care.

Balance and Fall prevention

Did you know that injuries from falling are the leading reason senior citizens require nursing home care? Or that most of those falls – 85%, in fact — occur at home?

Aging, physical changes, health conditions and even medications can make it more likely that someone will fall. Our balance and fall prevention program is designed to assess and minimize your risk.We consider physical issues like your balance, flexibility, muscle strength and gait as well identifying hazards in your home. Then we put into action an individualized plan to significantly reduce your fall risk.

Diabetes Management

Diabetes can be one of the most difficult chronic diseases for patients to manage, but controlling blood sugar levels in order to prevent serious complications is essential.

Through education and a realistic individualized treatment plan, our well-trained clinicians can make it easier for you to manage your diabetes. With their help and guidance, you’ll make better food choices, become more active, and be less likely to forget or overlook the important tasks of testing your blood and taking your medicine.
